• SDP Pre-hiring Packet

  • Criminal record statement

  • Criminal history statement

  • Confidential Employment and Personal Survey

  • Applicant information release

  • Request for Live Scan Service.​​

• Two forms of valid IDs e.g. social security card, driver’s license, passport.​

• Covid-19 Vaccine Record Card, or alternatively, submit a weekly Covid-19 test result

• If transportation is going to be rendered – ESP must complete the attached DMV form
